Entering Student Success Plans

| Reasons | Click to add a reason to be assigned to the student success plan. |

The SSP Plan Detail page includes sections named Plan Information, Reasons, Student Goals, Parent Goals, and Interventions.
Name of the staff member responsible for adding, administering, and tracking the student’s student success plan.
Date the plan was created or started. You cannot update the plan date for the existing plan.
Code and description of the success plan's plan type.
Title entered by the teacher for the plan.
The date the plan was completed.
Status of the plan. The possible statuses are:
C- Complete - The plan has been completed.
IP - In Progress - The plan is in progress.
PA - Pending Approval - The plan is yet to be approved.
R - Plan Rejected - The plan has been rejected.
Check to indicate that the plan contains sensitive information.
Text describing the need for assigning the plan to the student.
Text describing steps to be taken to complete the success plan.
Date the reason was identified for the student.
The reason code and related description that the teacher has selected. The teacher can enter reasons to be associated with the success plan.
Date the reason no longer applies to the student.
Enter a check and click (Save)
to remove the reason.
Code identifying a goal and its description.
Text describing the goal and its components.
Date the goal was completed.
Code identifying a goal and its description.
Additional comments entered by the teacher regarding the parent's goal.
Date the goal was completed.
Code identifying an intervention and its description.
Date the intervention started.
Date the intervention was completed.
Progress frequency set by the teacher. The possible frequencies are:
A - As Needed - The intervention can be tracked as needed.
E - Each Day - The intervention will be tracked each day.
W - Weekly - The intervention will be tracked weekly.
S - Selected Dates - The intervention can be tracked based on the specified date range.
